Abstract

Keselamatan pasien menjadi isu global yang menarik perhatian dunia kesehatan setelah The Institute of Medicine (IOM) menerbitkan buku berjudul “To Err is Human: Building a Safer Health System. To Err is Human.yaa.melakukan kesalahan adalah hal yang manusiawi, dan pelayanan kesehatan tentunya dilakukan adalah manusia, sehingga pelayanan kesehatan rentan terjadi kesalahan atau yang sering disebut sebagai kesalahan medik atau medical error.
Premis logis tersebut seolah didukung oleh data data ilmiah tentang jumlah kematian yang berkaitan dengan kesalahan medik. Data pertama masih diambil dari buku tersebut, dilaporkan bahwa di Amerika Serikat, sejumlah 44.000–98.000 kematian yang berkaitan dengan kesalahan medik terjadi setiap tahunnya. Angka ini lebih tinggi jika dibandingkan dengan jumlah kematian akibat driving, mountain climbing, dan yang lainnya.
